This is the Administrative Support Team's curated list of UW Procurement and Finance forms. To help you select the correct form, we've included short descriptions of each form.

Department Match Exceptions

Use this form to submit secure department match exceptions types of approval for other charges over $100, approval for freight over $1,000 or invoice line mismatches.

 

Manual Asset Registration

Request changes to an existing asset record in Workday that you are unable to make yourself. Or to request an asset be added to Workday when the item was not procured or constructed in Workday but needs to be tracked for compliance reasons.

 

New ProCard/Card Limit Increase

Apply for new ProCard, Ghost Account, Central Travel Account (CTA) and/or Individual Travel Card (ITC) cards, request limit increase or request Merchant Category Code (MCC) restriction lift.

 

Order Visa Reward Cards

Request one-time payment card for Human-subject research for UW Academy.

 

Procurement Contract Review

Use this form to request Procurement's assistance obtaining a contract or document review/signature, or if you need to initiate a solicitation.

 

Purchase Order - Change/Close

Use this form to change, close or cancel an existing PO.

 

Revolving Fund Account Management

This form is to be used to reconcile or replenish a Revolving Fund account. Revolving Fund bank accounts are used to pay research subjects and subject associated costs.

 

Sole Source Justification

Used to communicate to Procurement Services that: only one specific item or service can reasonably meet the department's need and/or only one supplier can reasonably provide that item or service.

 

Supplier Set-Up Request

Request set up and update existing suppliers in Workday.

 

Unauthorized Purchases

Use this form if a UW department (other than Procurement Services) enters into a contract, commits to purchases or makes purchases with one outside entity with cumulative purchases of $10,000 or more without Procurement’s documented pre-approval.
